Dementia is often thought of as a condition of old age, but new research suggests that some of the factors shaping risk may matter decades earlier.
A long-term U.S. study found that hypertension and diabetes around age 55 were associated with fewer years lived without dementia later in life.
The findings do not mean that avoiding these conditions guarantees protection from dementia. Genetics, age and many other factors also contribute. But the study adds to evidence that vascular health and brain health are closely connected.
<b>The important message is not that dementia can always be prevented, but that midlife health may influence how long cognitive health is preserved.</b>
<h3>More Than 12,000 People Followed</h3>
Researchers led by Dr. Josef Coresh at New York University Grossman School of Medicine analyzed data collected from four U.S. communities over roughly three decades.
The study included 12,409 people who did not have dementia at age 55 in the early 1990s. Researchers assessed whether participants smoked or had high blood pressure or diabetes at that point, then followed their health records through 2022.
Around 42% of participants had none of these three vascular risk factors. During follow-up, 3,008 people developed dementia, while 5,238 died without receiving a dementia diagnosis. The results were published in Neurology Open Access in August 2026.
<h3>Risk Factors Added Up</h3>
The clearest pattern appeared when researchers looked at how many vascular risk factors each person had. People with none of the three lived, on average, about 30 years beyond age 55 without dementia.
Those with all three lived roughly 17 dementia-free years after that age. That represents a difference of around 13 years.
<b>This does not mean that every person with hypertension and diabetes will develop dementia 13 years earlier.</b>
The figures describe average differences across a large population. They cannot predict an individual person’s future with the same precision. Still, the pattern suggests that accumulating vascular risks in midlife may be associated with both earlier dementia and shorter overall survival.
<h3>Why Vascular Health Matters</h3>
The brain depends on a constant supply of oxygen and nutrients delivered through blood vessels.
High blood pressure can damage those vessels over time. These processes may help explain why cardiovascular and cerebrovascular health are frequently linked with cognitive decline.
The study itself, however, was observational. That means researchers identified associations rather than proving that the three risk factors directly caused earlier dementia in every participant.
<h3>Women and Black Participants Differed</h3>
Researchers also observed differences between demographic groups. Women lived approximately 1.5 to three years longer without dementia than men who had the same number of vascular risk factors. Black participants, meanwhile, lived about three fewer dementia-free years than White participants on average.
These differences may reflect a combination of biological, social, healthcare and environmental influences, and the study was not designed to explain all of those factors.
<h3>Midlife May Be a Useful Window</h3>
Coresh has emphasized that the results strengthen the case for addressing vascular risks during midlife rather than waiting until older age.
He explained indirectly that maintaining healthier blood pressure and metabolic health may help preserve brain health for longer. That does not turn dementia prevention into a simple checklist. Some risk factors cannot be changed, and even people with excellent cardiovascular health can still develop dementia.
But blood pressure and diabetes are all factors that can often be managed through healthcare, lifestyle changes or both.
<b>The study’s practical lesson is therefore less about predicting dementia and more about protecting healthy years: what happens to blood vessels in midlife may matter for the brain decades later.</b>
